The Center for Spine & Pain Management provides evaluation and treatment of chronic pain and spine pain. Methods used include neural blockades, implantable drug delivery systems, spinal cord stimulation, and radiofrequency. Specializing in specific diagnoses or in pain related to a specific region of the body, the comprehensive Spine program puts the patient at the center of care. After evaluation and assessment are completed, patients can be treated with various therapies or interventional procedures, sometimes including surgery.

The Human Motion Institute is offering clinical expertise to establish an accurate diagnosis quickly and develop a plan of care that fits the needs of each patient suffering from a spinal injury.

If you are scheduled for an appointment in the Center for Spine & Pain Management, please download and print your intake questionnaire here. This questionnaire needs to be filled out prior to the time of your office appointment and takes approx 15-30 minutes to complete. Please bring it with you to your appointment to prevent rescheduling or delaying your visit.
